How to, and frequently asked questions from the

BHS Counseling Office

Ø      To obtain a copy of your transcript:  Please complete the Transcript Request Form and mail or bring it to the BHS Counseling Office.

Ø      To order a new diploma:  Complete the Transcript Request Form but make note on the form that you need a diploma.  Diplomas must be ordered; they take six to eight weeks, and cost $40.00.

Ø      To enroll a new student: 

1)     The student must be a resident in the Van Buren Public School District.  We are not a “school of choice” school.  The student must reside with a parent or a legal guardian.  If a student lives with a guardian, the guardian must provide court documentation proving guardianship.  If the guardian is a foster parent you must provide your letter from the agency placing the student in your care.

2)     The parent/guardian will need to bring 3 pieces of proof of residency in our district in the form of:

ü      Michigan Driver’s License (with current address), and

ü      Voter’s Registration, or

ü      Mortgage/lease agreement, or

ü      Utility bill with address, or

ü      Any other mail sent to you at your residence

3)     From the previous school you must obtain:

ü      “Unofficial copy” of the student’s Transcript (if 9th grade – previous year’s report card)

ü      Withdrawal Form

ü      IEP (if the student receives Special Services)

After the student is enrolled, we will request an “official copy” of the records, but you need the unofficial copy when meeting with the counselor to register the student.  After the school year begins, you will also need a “withdrawal form” with current grades-to-date.

4)     Up-to-date immunization record.  You can obtain this from your doctor’s office or possibly the previous school.

5)     Copy of the student’s birth certificate.

6)    There is also a packet of forms that needs to be completed by the parent/guardian of the student. These are available to print from the BHS Counseling Department webpage or in the counseling office. The documents take 10 to 15 minutes to complete.Completed forms can be returned to the counseling office from7:00 a.m.until 3:00 p.m. Monday - Friday. 

ALL of the above information is required.  We can not start the enrollment procedure without all of the information.  If you have any questions, please call our office.

 

Many parents ask us to fax the previous schools to obtain the above information.  We are sorry but we are unable to comply with this request for many reasons.  One in particular is that we (and you) become at the mercy of the other school’s time constrictions.  After faxing requests we have waited up to a week to receive the documents (even though the school says they will do it immediately).  Please take the time to visit the previous school and have the documents in hand when you arrive.  THANK YOU!
